To ask the Minister for Health the number of hazard, incident and near-miss reports indicating stress as a cause of the issuing of reports that were submitted, by hospital, in each of the years 2011 to 2016 to date, in tabular form, under the Health Service Executive’s policy for preventing and managing stress in the workplace.
