I propose to take Questions Nos. 1297 and 1298 together.
The Mental Health Commission has confirmed to my Department that it has been in communication with the family of the person since May 2025. Correspondence was sent to the family on 9 June, 7 July, 28 August and 11 September 2025. The MHC outlined that it does not have the authority to investigate family/patient complaints. It stated that this is a matter for the HSE. Therefore, the MHC advised that the HSE will be best placed to outline what investigations it has carried out in relation to the death of the person.
I have asked the Health Service Executive to respond directly to the Deputy as soon as possible.

To ask the Minister for Health if she will confirm whether Garda letters and search warrants were requested by or supplied to the HSE/Central Mental Hospital for risk management of a person (details supplied); and if not, the reason Garda homicide-risk warnings were excluded from the persons medical record and care planning.
