To ask the Minister for Children, Equality, Disability, Integration and Youth if she will commission an independent report into the death of a person (details supplied) under the care of an organisation given that the report commissioned by the organisation made no finding of liability or failure in duty of care despite that later, as a result of a civil action, the organisation admitted liability. I extend my sympathies to the family of the young girl who lost her life so tragically. I appreciate the enormous trauma that has been visited upon the family. I understand that the events leading to the tragic death of this young girl have been the subject of court proceedings, leading to a settlement by the youth organisation under whose watch they occurred.
