My Department provides multiple channels for the public to access Social Welfare Application forms:

1. In person - by visiting their local Intreo Centre
2. Online – application forms can be downloaded from Gov.ie website. Alternatively, people can use our “Request a Postal Form” which is also available on the Gov.ie website.
3. By phone – by contacting the National Intreo Contact Centre for Intreo scheme forms or by phoning other scheme areas directly.
4. An Post – a selection of Social Welfare application forms is available in each Post Office.
