SI No. 45/2021 provides the legislative basis for the COVID-19 Passenger Locator Form and sets out an offence where the form is not completed in the manner specified or where false or misleading information is provided on the on the form. These offenses are punishable by a fine not exceeding €2,500 or imprisonment for a term not exceeding 6 months, or both.
