The Department has received a medical certificate from the person concerned, however, an application form for Illness Benefit (IB1) has not yet been received from him.

An application for Illness Benefit form has been posted to the person concerned for completion and his claim will be processed as soon as it is received by the Department.

I trust this clarifies the matter for the Deputy.
