To ask the Minister for Health the reason a person (details supplied) failed to investigate or have investigated the disclosure that over 15 patients had a delayed head and neck cancer diagnosis despite the fact that the clinical incidents had been placed on Q-Pulse by a senior clinician; and the reason the person has failed, neglected or omitted to investigate or have investigated over 30 other serious incident submissions by a consultant ENT surgeon.
