To ask the Minister for Health the number of national incident report forms that were filed over the past four years, respectively, including 2018, in which death was registered as the outcome of self-injurious behaviour, including a breakdown on the basis of facility by CHO area; and if the behaviour was logged as intentional or unintentional.
