The Integrated Care Programme for the Prevention and Management of Chronic Disease, in partnership with the National Clinical Programme for Heart Failure, is planning to conduct a service gap analysis to inform integrated care projects to address service gaps for heart failure.

This will include specialist ambulatory care and hospital inpatient specialist care.  A report on the outcome of the gap analysis is expected to be presented to the HSE Design Board by end 2018.
