I have been advised by the military authorities that a Certificate of Service (LA 89) was issued in respect of the individual referred to by the Deputy.  However, it is not possible to provide the individual concerned with a replacement copy, as a Certificate of Service is issued on a once off basis and cannot be replicated. A Statement of Service in the Defence Forces (AF 643) can however be reissued. In this regard, I have been advised that the relevant military authorities will complete a Statement of Service in respect of the individual concerned and issue it to him at the address that was provided by the Deputy.
