Ms. Kennedy’s FIS claim is due to expire on 11^thOctober 2017. A FIS renewal form was issued to Ms. Kennedy on 3^rdAugust 2017 but has not yet been returned.Another FIS Renewal form issued to Ms. Kennedy today 10^thOctober 2017.

On receipt of the FIS Renewal form Ms. Kennedy’s entitlement to FIS will be decided.
