Correspondence has been received by my office in relation to this matter. I also received correspondence about this matter in 2016.

I am advised by the HSE that in this case there was an allegation of a safeguarding issue. This allegation was screened by the individual’s service provider who concluded that there was no cause for concern. The Safeguarding Team in the HSE also reviewed this case and concurred with the provider’s findings. As the individual’s family remain concerned, the HSE Safeguarding Team is initiating a further screening to adequately record accounts from all family members. I am assured that all appropriate safeguarding measures have taken place in the interim, for people receiving the service.

I am informed that while the Chief Officer accountable for the area is satisfied with the findings of both the service provider and the HSE’s Safeguarding Team, the National Director for Quality Assurance and Verification Division, HSE, in consultation with the National Director for Social Care Division, HSE, has appointed the Chairperson of the National Independent Review Panel to review the file on this matter and she is expected to provide a report on the overall response to the concerns raised.
