To ask the Minister for Health if he is aware of the instruction on a website (details supplied); the way in which the HSE has implemented this instruction; and the number of girls in each of the years 2011 to 2015 who where deemed unsuitable for the vaccine on the grounds that it was unsafe, with reference to these reactions after the HSE forms were completed and returned by the parents to the schools.
