To ask the Minister for Health the stage the second inquiry into an incident (details supplied) is at; if the second report is completed; if it has been provided to the parents; if the report and its recommendations will be published; the status of employment of the consultant obstetrician involved; and if he will make a statement on the matter. At the outset, I am conscious of the personal tragedy of the family at the centre of this sad incident and would like to offer my sincerest sympathies to them on their loss.

As this is a service matter, it has been referred to the Health Service Executive for attention and direct reply to the Deputy. If the Deputy has not received a reply from the HSE within 15 working days, I would ask him to please contact my Private Office and my officials will follow the matter up.
