243. To ask the Minister for Health to outline the full facts in relation to the number of meetings of the team appointed by the Health Service Executive to address the outstanding issues that arose following the death of a person (details supplied) and the dates of those meetings; the documents which were given to and created by the team while the consultant was a member; the issues addressed while the consultant was a member; to provide the details surrounding the withdrawal of the consultant from the team and the reasons for same; the meetings held by this team without the consultant after their withdrawal and what reports or comments were made either verbally or in writing to the Health Service Executive following the withdrawal of the consultant, and actions taken as a result; and if he will make a statement on the matter.
