515. To ask the Minister for Health the reason why the application form available for download on the long-term illness scheme section of the Health Service Executive website has yet to be amended to reflect the rewording of one of the qualifying conditions to the correct term of "intellectual disability"; when this change will occur; if he is aware that this amendment has yet to be formally completed despite several months of this Deputy having raised it through various means, including through Parliamentary Questions; and if he will make a statement on the matter. Under the Health (Pricing and Supply of Medical Goods) Act 2013, the HSE has statutory responsibility for the administration of the primary care schemes, therefore the matter has been referred to the HSE for attention and direct reply to you.

If you have not received a reply from the HSE within 15 working days please contact my Private Office and they will follow up the matter with them.
