To ask the Minister for Health to set out the detail on how home care packages are put together by the Health Service Executive; the services provided by health care professionals in 2015; the agencies involved in the provision of these professionals; the average cost of the package; if these professionals are co-ordinated in community settings or centrally within the Executive; and if he will make a statement on the matter.
