I can confirm that the department is in receipt of an application for disability allowance from the above named person. On 16th September 2015, the person concerned was requested to supply outstanding information required by the deciding officer in order to make a decision on the application. On receipt of this information a decision will be made and the person concerned will be notified directly of the outcome.
