To ask the Minister for Health given the supervisory review of the practice of a midwife (details supplied), claimed by him as a measure to expedite the parallel systems analysis investigation, whether a final report was issued by the supervisory review team to the Health Service Executive; if so, the date on which it was issued; and if not, the reason for the failure to do so.
