To ask the Minister for Health the reason copies of the two most recent health and safety reports into the psychiatric unit in University Hospital Galway have not been provided to all those legally entitled to receive them; if disciplinary action has been initiated against the Health Service Executive managers for any failure that may have arisen with respect to their statutory responsibility to provide said copies; and if he will make a statement on the matter.
