718. To ask the Minister for Health the position regarding the report prepared by a team from the National Maternity Hospital into an obstetric clinical incident in Cavan General Hospital in November 2012 that resulted in the loss of a newborn child; with whom the report is currently; when will the parents be made aware of its content and recommendations; if the report will be published and, if so, the date of same; and if he will make a statement on the matter. Following an obstetric clinical incident in Cavan General Hospital in November 2012, an external review was undertaken by a team from the National Maternity Hospital. The external review team examined the clinical management and care given to an obstetric patient and her baby, (in line with HSE Incident Management Investigation policy). My Department understands that the external review team has submitted a draft report to the HSE. This report is now subject to due and fair process so as to ensure the matter is addressed appropriately.

The HSE remains fully committed to bringing a satisfactory conclusion to this matter as soon as possible. When the final report is complete it will be given to the family involved.  It is expected that this process should be completed by the end of July 2014.
