The above named is currently in receipt of Disability Allowance from my department. However, there is no record of a recent decision on carer's allowance ever having been made in respect of her entitlement to carer's allowance at half rate in conjunction with her disability allowance.

On 5 June 2012, an incomplete claim form, received from the above named was return to her for completion. Upon its completion and return to carer's allowance section, her application will be processed.
