Rent supplement application forms are available from local Community Welfare Officers in Health Centres, and are also available to download from www.communitywelfareservice.ie. Clients are also welcome to call the freephone number (1800 201 698) and request an application pack to be sent to them. The HSE has advised that an application pack has been posted to the person concerned at the address given.
