Question : To ask the Minister for Health and Children if she will instigate an independent inquiry into the misdiagnoses of breast cancer in Ms Edel Kelly and Ms Ann Moriarty, which in both cases occurred in Ennis General Hospital. Furthermore, such an investigation should include the mammogram which was carried out at St James's Hospital and the core biopsy that was carried out at Limerick General Hospital. In that way we can reassure the public that our health services are indeed safe and that any lessons we learn from this will result in the implementation of a proper system to ensure that human error does not impact negatively on patient care.
