I seek the adjournment of the Dáil under Standing Order 31 to raise a matter of national importance, namely, the need to establish an independent inquiry into the death of a long-stay psychiatric patient at St. Brendan's Hospital, Dublin, on 1 September who, due to the side effects of medication, choked to death on her food.
