In that case we had to go to the laboratory and make a number of escalation calls to have the slide released. Its excuse at the time was pressures of contracts, backlogs and other factors. It had a legal case pending. We held an escalation meeting with the laboratories on a fortnightly basis to review records, slides and other pressures to ensure we would trap them much earlier in terms of requests made to the laboratories.
