In my experience, mostly it is. It depends on how an adverse outcome is interpreted. There are several interpretations. When a case goes to court we hear two very different sides to it. Open disclosure may present a certain number of facts but they may not be entirely the same as if somebody else looked at it. However, they are true to the person presenting it to the patient.
