As I understand it, nothing happened at acute hospital level for 15 months. Dr. Hickey then elevated it within the hospital. They had not any idea about this because as we now know from Dr. O'Keeffe, they were not copied on the 16 June documentation. To be fair, they did not know about it, which is amazing. The seven group hospital managers in the country were not made aware of it. Does Dr. Henry not feel that as an adviser to the acute directorate there should have been some form of follow-up? This had to be escalated. If there was such a concern in one hospital group, and we see from the letters that they were deeply concerned, surely at management level, and I know Dr. Henry is on the clinical side, there should have been a follow-up action. If Mr. Woods was here we would ask him, but he is not.
