Even in this instance, we, in good faith, identified issues, surfaced the issues and put a process in place to deal with them. The finger is then pointed - it is not just this particular issue; I can use a multitude of examples - and we are asked how come it happened on our watch. On our watch, these matters were identified and surfaced and are being dealt with.
