To ask the Minister for Health the involvement of bereaved families in HSE internal reviews into baby deaths, maternal deaths and catastrophic birth injuries; the steps taken to ensure that these cases are dealt with in a timely manner in safety incident management team reviews; if there is any available data in respect of these time frames; the person or body responsible for oversight of these reviews; the repercussions for non-adherence; and if he will make a statement on the matter.
