To ask the Minister for Health the number of deaths in approved centres and mental health services reported to the coroner for the past ten years; the follow up procedures when a death is reported in an approved centre; if records of findings are maintained; and if he will make a statement on the matter. As this is a service matter, I have asked the Health Service Executive to respond directly to the Deputy as soon as possible.
