With regard to the cervical smear testing, the lessons the SCA learned, when it became aware of the issue and how it will avoid similar issues in the future, how did it happen that the matter did not go on the risk register sooner?

I am saying to the witness, as the chief doctor, that the message in this memo has nothing to do with difficulties with cervical smears.
