Prior to the establishment of the HSE, the health boards were the agents of the State. Who is culpable for allowing that non-standardised approach? Jerry Buttimer: Why did it take so long for the information to come to light? Let us take the example of the HIQA investigation which brought the information to light, if it is as hands-on as Ms McGuinness suggests, how did we allow a situation to evolve where there was a piecemeal approach taken and suddenly we are in the middle of a crisis, which does nobody any good?
