Marian Harkin: My question is more about timing. This happened quite some time ago. Members of a surgical team had to have known. It could not have happened in front of their eyes without some of them knowing. Maybe management did not know. Has there been any review as to how this could have happened? Is the Minister just saying that the hospital is only reviewing the matter now?
