Colm Burke: I know all of that. The concern I have is that this was all the more reason to have checks and balances in place. However, there were no checks and balances for four years.

I am saying I am not satisfied in the sense that there were no checks and balances, particularly given that there was a junior doctor doing the work that should normally be done by a consultant. That was all the more reason that there should have been additional checks put in place.
