Michael Harty: We did not mention ehealth and the single patient record. A patient may be admitted to St. Vincent's Univeristy Hospital, for example, discharged and then admitted to Loughlinstown or some other hospital but with no notes or other communication. The GP often has to be consulted on the patient's background.

Michael Harty: To come back to my original question, apart from staffing and bed capacity, what are the barriers to implementing the chronic care programmes? Are there structural problems that need to be overcome in how the HSE and the Department of Health are structured and managed or are there other softer barriers apart from the infrastructural and staffing barriers?

Michael Harty: Where does the initial impetus come from to set up local integration groups? Does it come from the hospital sector or from general practice or do the groups come together and decide to do it?
