Colm Burke: If what I am saying is correct, is there not a danger now for everybody who identifies a particular problem that they will be pushed to the side and told they are no longer wanted around? That seems to be the message coming back on this.

Colm Burke: I know all of that. The concern I have is that this was all the more reason to have checks and balances in place. However, there were no checks and balances for four years.

Colm Burke: We have gone over all of that. I am saying I am not satisfied in the sense that there were no checks and balances, particularly given that there was a junior doctor doing the work that should normally be done by a consultant. That was all the more reason that there should have been additional checks put in place.
