Catherine Murphy: Mr. Walsh is the chair of An Bord Pleanála. The buck stops with him in terms of making sure these things are in place. Some of it is open to individuals having the integrity to declare while other things were not spotted. This screams of systems failure.

In Mr. Walsh's time as the chairman of An Bord Pleanála have there been any instances where the number of people making those decisions has been fewer than three?
