I am not going to root out an individual on the basis that this was a case of staff being overworked and a number of systems failures. Rather than a culture of blame, I would rather a culture of people being free to put up their hands if an error happens - we all make mistakes - and then making sure that we learn from such mistakes and put in robust systems to make sure they do not happen again.
