I apologise if I am asking bullet like questions, but it strikes me that there is a significant amount of firefighting. Ms Creamer mentioned the policies and procedures all organisations should have in place. Is there any monitoring to ensure organisations are sticking to those policies and procedures? This issue would probably not have come to light were it not for the emergence of the retrospective issues.
