That would certainly be the focus but I would not focus on whether individuals had some responsibility in that regard. My focus would be on functions, committees and whether those committees worked. The kind of things one would look for would be situations in which a committee was supposed to operate but did not operate or did not get some information it should have got and did not seek it. It could also be a case of determining, if an early mistake was made, the point at which the controls that were in place picked up on it. These are the kind of things one would look for. I expect that the work PwC has been commissioned to carry out, as reported here, will address some of those points.
