Somebody needs to be held accountable for it. I am asking about whose responsibility it was to ensure that loop was closed. Mr. Gleeson feels it may or may not have been his responsibility. He is not sure, even to this day. Dr. O'Keeffe is saying it was the responsibility of the person under her to feed that information up to her.
