As a case study, it shows that there are clear process failures. In my view, a number of different agencies did not act in the way we would expect them to. The Family Support Agency is gone as it is subsumed into Tusla. In view of this we should write to Tusla at the very least and outline our concerns on this matter and on how there is a difference between reporting a complaint and making a complaint.
