Finally, I would like to point out that we have improved our own forms, which have come up a couple of times. Besides the medical application form, we also introduced form MR99 in 2012.

The latter is sent to the claimant in order that he or she can outline how his or her condition affects him or her in terms of activities and daily living and the impact it has on his or her faculties. This is a very useful form. In the context of the domiciliary care scheme, we recently introduced the Dom Care 3 form. This is issued to the care provider - the medical person - and he or she uses it to provide a comprehensive report on the person being cared for.
